Self-certification (SC2) forms can be completed for illness lasting 7 days or less.
These are available from employers or the HMRC website
If self-certification is not acceptable to your employer, a Private Medical Certificate can be issued
Illness more than 7 days
Employers may ask for medical evidence to support Statutory Sick Pay (SSP).
The 'Statement of Fitness for Work' Medical Certificate provides strong evidence and would normally be accepted, unless there was evidence to the contrary.
'Statement of Fitness for Work' - 'Fit note'
From 2010, this note can be used to help those patients keen to get back to work with the support of their employers by giving more information regarding the impact of their condition.
